PART 1 -

Form - Using Notepad, Word, FrontPage or whatever, create a Web Page with one question with each of the following types of Forms:
- Radio buttons,
- Check boxes,
- Drop down boxes.
- One-line text box.
- Multi-line textblock
- Submit and Reset buttons.

PART 2a - Static Excel Database: Build a static Web page table from a 4 row by 4 column Excel (or comparable) spreadsheet.

PART 2b -

Static Access Database: From an Access (or comparable database) table of at least 4 columns by 4 rows you create generate and link with a "Home Page" (or Switchboard Menu) each of the following:

A Static table in HTML from a static Web page query from that Access database,
A static Web page Form, A
A static Web Page report from that Access database, and link these with a "Home Page" (or Switchboard menu). As usual, link these Web pages to your MIS 480/680 Web page.

PART 3 - IDC/HTX:

Add your name, an address, a phone number and a comment (if required) using theIDC/HTX Customer Add form.
Run the Customer List Query that shows your addition and attach to your MIS 480/680 Web page as evidence for this assignment.
IDC file that specifies ODBC Data Source and SQL query built by hand or using Access - Save As HTML.
HTX file that specifies layout of resulting table.

PART 5 - Dynamic Access Database: - Download the Meals Access Database (340 KB) to your hard drive or floppy. - - Build a dynamic IDC/HTX or ASP query of a table or query in the Meals database. - - Email the IDC and HTX or ASP files to tower@oakland.edu.

- - Tower will load on "http://tower.sba.oakland.edu/mis480/ex10/" Web site server. - - Link to your MIS 480/680 site so you can execute it.
